Presentation of Evidence Based Practice Change Proposal: Create a Power Point of approximately 16 slides. .
Include the following: title , triggering issues, home care facility, purpose, priority and need for the change, team members, can inlude doctors, nurses, social worker etc, disciplines represented & expertise, patient preferences, search details (PICOT question, databases searched, key words used), synthesis of the evidence, design for implementation, design of a pilot test, resources available, barriers or constraints, how you will promote its adoption and disseminate if successful, and references.
PICOT Question: How cognitive behavior therapy can improve Insomnia and anxiety in patient with PTSD.
