The Analysis phase of DMAIC is designed to use the information from the Define and Measure phases to determine if there is a hypothesis regarding why the late lab result problem is occurring. One of the easiest methods to use to analyze information in this step is a Cause and Effect Diagram (commonly called a Fishbone Diagram or an Ishikawa Diagram). What are the steps necessary to develop a C&E diagram listed in your text and give an example of how you may use this for our lab result delay issue.
