ideally, you will use the pt from the last assignment and work with him/her to write 5 smart goals he/she could use and you could assist them with during a long recovery process. Please provide a short description of the person you are working with on these goals. Ie. their condition, state of mind, maybe their physical abilities. These goals are NOT to be physically focused, they are to be psychologically focused. These goals need to be in the SMART format and should be labeled as such.
